THE END
So basically Jo's story wraps up like so:
She stays behind with Kate, Sawyer, Ben et. al. when Jack, Locke and Desmond go on to the heart of the island (after a tearful goodbye with Jack, naturally, since neither of them really knows what will happen). She and Ben talk and actually, finally, make peace with one another. The groups meet up again on the cliff, where Locke dies, and Jo chooses to stay with Jack, Hurley and Ben as they send Kate and Sawyer off and return to the Source. Another tearful goodbye, this time for real because she knows Jack is dying. Ben holds her back while Jack transfers guardianship of the island to Hurley. One last tearful kiss before Jack is lowered into the Source, and a reluctant promise that she'll move on without him. She is hopeful for a moment when she thinks Jack is coming back to her, but instead it is Desmond. Jo stays with Ben, Hurley and Desmond and they move into the Barracks temporarily, until Hurley decides how to enact his Guardianship. Desmond promises to stay with her and keep her safe until they can both leave the island together. She swears she won't leave until she knows where Chase is, and Desmond waits with her.
Chase, meanwhile, stayed behind on Hydra Island, opting out of Lapidus' flight off the island because he wants to find Jo. He takes an outrigger to the main island, finds his way to the Barracks and reunites with Jo. She tells him the happy news about her pregnancy. Hurley and Ben arrange for Jo, Desmond and Chase to leave the island permanently. Before she leaves, Jo makes her way to Boone's grave to say a final goodbye. Jo and Chase resume normal life back in L.A., have a baby named Juliet, and return to the Diagnostics Dept. of St. Sebastian hospital.
In the sideways world, Jo reunites with her soulmate outside the church (as already posted, see above), but this time he is dramatically revealed as: BOONE! They enter the church together and are happily reunited with their best friends and soulmates.

It was all much more dramatic in my head when I imagined it, immediately after watching the finale, but life got in the way and kept me from writing. I know I'll never get around to finishing it off properly, so this is as good as it gets. Now I've got closure (and a probable readership of zero!), hoooray!! ;D
OH yes, and I chose Boone, because Jo has pretty much been pining for him since he died, even though their romance was only just beginning to spark when tragedy struck. Even though the character isn't wonderful, Ian Somerhaulder has gotten pretty damn hot in his role on the Vampire Diaries, so there's another reason! And if Sayid can end up with Shannon, instead of his REAL soulmate Nadia, then there's some flexibility with the storyline there!
